Maybe it's just me, but every time I try to read the "toggleMode" method
in the audio output example, I get confused on whether I'm switching to/
from push/pull mode, and even what pushing and pulling entails. The name
of the push timer also seemed backwards (which added to the confusion).

Qt copyrights are now in The Qt Company, so we could update the source
code headers accordingly. In the same go we should also fix the links to
point to qt.io.

Having a code snippet use a slot (stateChanged) with a name
identical to a signal in QAudioInput / QAudioOutput classes causes
problems (incorrect links) in class documentation that refer to
the snippet code.
This change renames the slots according to standard Qt convention.
Same change is applied to example applications as well.
