Qt copyrights are now in The Qt Company, so we could update the source
code headers accordingly. In the same go we should also fix the links to
point to qt.io.

The logic for the implementation is taken from the Android plugin.

This patch also makes sure AVF video layers are removed from their
parent layer when their corresponding Qt video outputs are destroyed.
